MAKE THE SWITCH

Anyone with an NDIS Plan can choose to use plan management.

This plan management request can be made at any time: at an NDIS planning meeting or at any stage during the life of an NDIS plan. Here’s how to make the change in three simple ways:

1. Call the NDIS on 1800 800 110 and make a plan management request; or

2. Download and complete a Change of Situation/Change of Details form from the NDIS found here. Complete the form, including Part C and Part E and send the form back to the NDIS (the email and address details are on the form); or

3. Simply fill in our intake form found here or contact Nicole and Sarah via mobile at 0411 963 600 from Monday to Friday between 8am - 4pm.
